How to Write an Outstanding Letter of Recommendation (LOR) for a student or resident.

Why Your Letter Matters

A letter of recommendation (LOR) is not a formality, it’s a powerful narrative that can open doors for a medical applicant or quietly close them. In residency and medical school applications, a generic letter is often worse than none at all. Programs are inundated with “good” applicants. What they seek are great ones. A strong LOR doesn’t just confirm competence; it provides compelling evidence of potential.

If you cannot write an outstanding letter, you must decline. This isn’t cruelty, it’s professionalism. A tepid or vague endorsement can be a quiet disqualification.

Before You Say Yes

Before agreeing to write a letter, ask yourself:

  • Do I know this applicant well enough to describe them specifically?
  • Have I observed their clinical or interpersonal skills firsthand?
  • Can I truthfully say I would want them in my program or caring for my loved ones?

If the answer to any of these is not a clear yes, decline with kindness and integrity.

The Anatomy of an Outstanding Letter

1. Start with Your Identity and Relationship

Set the stage with:

  • Your name, role, and institution
  • Your specialty and academic rank
  • How you know the applicant and for how long

Example:
“I am Dr. Hans Gruber-Morris, a board-certified internist and associate professor of medicine at Cityview Medical Center. I had the privilege of supervising Ms. Priya Shah during her internal medicine sub-internship from January to February 2025.”

2. Assess Interpersonal and Professionalism Skills

Describe:

  • How they communicate with patients and staff
  • Their maturity, empathy, and reliability
  • How they function on a team

Avoid vague praise. Instead, illustrate with a moment or pattern of behavior.

Example:
“Priya’s ability to listen without interruption and de-escalate anxious families was unmatched among her peers. A nurse once remarked, ‘She doesn’t just calm the patient, she calms the room.'”

3. Highlight a Clinically Defining Moment

Tell a story that shows clinical skill or potential:

  • Diagnosing an obscure condition
  • Managing a difficult case
  • Demonstrating initiative or poise under pressure

Example:
“During a busy overnight shift, Priya correctly identified early signs of spinal epidural abscess in a patient admitted for back pain—a diagnosis that had been missed by multiple providers. Her prompt recognition and escalation prevented devastating neurologic consequences.”

4. Describe Knowledge Sharing or Teaching Ability

Programs want team players who elevate others. Mention:

  • Informal or formal teaching of junior learners
  • Clear communication of medical reasoning
  • Willingness to mentor or explain

Example:
“She often led chalk talks for third-year students, distilling complex nephrology topics into memorable frameworks. Her ability to teach revealed not only knowledge but genuine enthusiasm for medicine.”

5. Summarize Core Clinical and Personal Strengths

Briefly recap the applicant’s most important traits:

  • Clinical reasoning
  • Compassion
  • Resilience
  • Intellectual curiosity

Example:
“Priya is curious, grounded, clinically sharp, and emotionally intelligent. She has every attribute we hope for in a future internist.”

6. Close with a Clear, Strong Endorsement

Never write: “I have no reservations.” It sounds like you barely remember the student.

Instead, make it personal and strong:

  • “I would gladly have them in my program.”
  • “I would trust them to care for my own family.”

Example:
“I recommend Priya with the utmost confidence. If I were running a residency program, she would be among my top recruits. I would trust her completely with the care of my own family.”

Final Guidelines

  • Specificity is everything. Bland praise reads as faint praise.
  • Avoid recycled text. Letters that sound formulaic damage credibility.
  • Proofread. Grammar errors reflect poorly on both you and the applicant.
  • Include contact information. Make it easy for programs to follow up.

When to Say No

Declining a request is not unkind—it is professional.

Suggested response:
“Thank you for asking. I appreciate the opportunity, but I don’t feel I know you well enough to write the kind of strong letter your application deserves. I want you to have the strongest support possible.”

Final Thought

If your letter doesn’t create a vivid, unmistakable impression of excellence, it may harm more than help. A great LOR takes effort, but it’s worth it.
